   Use this form when reporting one of the following issues:

   1. E-Collection in NZ is not set up to be shared (Library does not appear in "Available For" or "Held by")

   2. Other NZ e-collection issues or questions

   3. Shared portfolio is missing from e-collection

   4. Coverage date for shared portfolio is incorrect

   5. Unable to access full text for a shared e-collection (include detailed description below)

   6. E-Collection in NZ not linking to resource (not recognized as subscribed institution)
